TRAVEL WAIVER/ HOLD HARMLESS AGREEMENT
  • I give my permission for my student to participate in the above activity. 
  • I acknowledge that the HBCU Climate Change  Consortium has offered to provide transportation to and from the Activity. 
  • I understand and acknowledge that serious accidents sometimes occur during such travel, and that that the travel could result in loss of or damage to property and  injury to the student. 
  • I am aware of the inherent potential risks associated with such travel and am willing to assume these risks. 
  • I understand and acknowledge that the HBCU Climate Change Consortium accepts no responsibility for the student’s travel injuries or damages resulting therefrom are not covered by any HBCU Climate Change Consortium insurance policies. 
  • In consideration of my  participation in the event, on behalf of the student, my family, heirs, and personal  representative(s), I hereby release the HBCU Climate Change Consortium, its  governing board, officers, employees, and representatives (collectively the  “Releasees”) from any and all liability for any and all claims and causes of action for  loss or damage to the student’s property, personal injury or death, that may result from or occur as a result of my travel. I further agree to indemnify and hold harmless the Releasees from liability arising from my tortious acts or omissions. 
  • I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND IT TO BE A RELEASE  OF ALL CLAIMS AND CAUSES OF ACTION FOR THE STUDENT’S INJURY OR DEATH OR  DAMAGE TO PROPERTY THAT OCCURS WHILE TRAVELING TO OR FROM THE  ACTIVITY AND IT OBLIGATES ME TO INDEMNIFY THE PARTIES NAMED FOR ANY  LIABILITY FROM MY TORTIOUS ACTS OR OMISSIONS. 

  • I further agree that this Release shall be construed in accordance with the laws of the States of Louisiana, Texas, Florida, Alabama, Mississippi and Michigan. If any term or provision of this Release shall be held illegal, unenforceable, or  in conflict with any law governing this Release the validity of the remaining portions  shall not be affected thereby. 

MEDIA RELEASE
I, as the parent or guardian of the student listed above, hereby give the Deep South Center for Environmental Justice (DSCEJ) and its employees, representatives, and authorized media organizations permission to print, photograph, and record my child for use in audio, video, film, or any other electronic, digital and printed media.
  • This is with the understanding that neither DSCEJ nor its representatives will reproduce said photograph, interview, or likeness for any commercial value or receive monetary gain for use of any reproduction/broadcast of said photograph or likeness. I am also fully aware that I will not receive monetary compensation for my child’s participation.
  • I further release and relieve DSCEJ, its employees, and other representatives from any liabilities, known or unknown, arising out of the use of this material. I certify that I have read the Media Consent and Release Liability statement and fully understand its terms and conditions.

Led by environmental justice scholar and advocate, author, civic leader and professor of Sociology Dr. Beverly L. Wright, the DSCEJ uses research, education, and community and student engagement to advocate for policy change, lead health and safety training for environmental careers, develop social and emotional community wellness programs, and create new and environmentally healthy opportunities for the residents of communities disproportionately impacted by historic environmental injustice.
